You may have visited a Quest Diagnostics Patient Service Center location or Quest Diagnostics performed your lab testing at your doctor’s office. At Quest, we know how important lab testing is to your health. We also understand how important it is to know how much your testing will cost.
- Your lab testing with Quest will be billed separately from your doctor visit
- Depending on your health plan coverage, you may receive a bill from Quest for the deductible, copay or co-insurance for your lab tests
When Quest performs lab testing ordered by your doctor:
- We file a claim with your health insurance provider.
- Your health insurance provider will pay us based on your coverage. If your health plan does not cover the full cost of the testing, you may get a direct bill from Quest Diagnostics.
If you receive a bill from Quest:
- You can pay online at QuestDiagnostics.com/Bill or by calling 1.800.488.8890
- You can sign up for paperless billing at QuestDiagnostics.com/GoPaperless
Here are some of the terms your health plan might use to
describe what you’ll pay for lab testing.
Deductible: What you have to pay before your health coverage kicks in
If your deductible is $500, you must pay $500 out-of-pocket before your plan begins paying for lab tests or other services.
Co-pay: What you pay for each lab test, doctor visit, or other service after you’ve met your deductible
Remember, lab tests are billed separately from doctor visits. For example, you might have a $30 co-pay for a lab test and a $25 co-pay for a doctor visit.
Co-insurance: The percentage you pay for a service after you’ve met your deductible
Co-insurance is different from a co-pay. Instead of a dollar amount, your health plan might cover 80% of a service, and require that you pay the remaining 20%.
