Quest Diagnostics is partnering with Choose Healthy Life to implement a SARS-CoV-2 (COVID-19) testing program in your community. The testing program consists of SARS-CoV-2 (COVID-19) molecular (PCR) testing for active infection. This page will help you register for testing.

If you are seriously ill, please seek immediate medical care from a healthcare professional.
If you are having a medical emergency, please call 9-1-1. Signs of a serious medical condition include, but are not limited to, severe shortness of breath or difficulty breathing, coughing up blood, chest pain, irregular heartbeat, persistent vomiting, or diarrhea.
How to get started
- Click the "request a test" button to begin.
- In order to receive your test results, you will be asked to create a MyQuest™ account. Creating an account is free and secure. If you already have a MyQuest™ account, you can log in with that.
- Once you have registered for testing, your onsite Health Navigator will let you know what to do next.
